The archive comprises a circular issued by the Union of Welsh Baptists in 1936 to the Baptist chapels; the completed questionnaires; a list of the chapel records collected; a list of the chapel records compiled from the questionnaire (8 copies); a list of the pre-1837 records compiled from the questionnaire.

A printed questionnaire, containing lists of various editions in Welsh of the Bible and of portions of it, the Book of Common Prayer, the prose Psalms, Psalters, etc., circulated by John Ballinger (1860-1933) in 1903 on behalf of the Cardiff Public Library, with annotations by David Richard Thomas (1833-1916), archdeacon of St Asaph.
